Your First Visit

Physical Therapy

On the road to recovery our licensed therapists will be there

to help you move forward.


First Physical Therapy Appointment

At our office our we want to get you off to a good start and see you through the entire process of rehabilitation. The information below provides details for every step in becoming a new physical therapy patient at our center.  Your initial physical therapy appointment, including registration, evaluation, treatment and follow-up planning is typically 60 minutes.


Registration

      Please plan to arrive at your appointment about 15 minutes ahead of the scheduled appointment time to complete
      paperwork. For your convenience, you can complete some of the paperwork in advance by printing forms from       this web site and bringing them with you.

      We suggest you wear comfortable clothing and low heeled shoes.

      Please bring:
              •
Insurance card(s)
              • Prescription for physical therapy
              • Referral, if your insurance requires a referral

      If you do not have a doctor’s prescription, you may be eligible to see a physical therapist under NYS provisions       for Direct Access, up to a total of 10 visits within 30 days. These visits may or may not be covered by your       insurance. Our office staff will coordinate with your insurance company to determine if they will provide coverage.       

      If this is a Worker’s Compensation case, bring all details relating to this incident i.e. date of injury, claim number,       case manager’s contact information. If this is a Motor Vehicle Accident (No Fault), bring all details relating

      to this accident i.e. date of accident, auto insurance information, attorney lien information. 

      Finally, be prepared to pay your co-payment or coinsurance payment, if required. Co-payments or

      coinsurance payments are payable as you check in for each physical therapy appointment.

Evaluation

      The therapist will discuss your medical history, current problems/complaints, pain intensity and what aggravates       or eases the problem. Daily activities or your functional limitations, medications, tests and procedures related to       your health as well as your physical therapy goals will be discussed.  After a thorough evaluation, the therapist       will formulate a list of problems and a suggested course of treatment.

Treatment

      Treatment will be provided for your particular diagnosis as part of your first appointment.  Learn more about the       types of physical therapy treatments our therapists specialize in.

Follow-up Planning

      At the end of treatment, the therapist will develop your care plan including the frequency and duration of your       follow-up physical therapy visits.  At this time you may be given a home exercise program. The therapist will send

      a consult to your physician summarizing your visit and the planned treatments. The doctor will have the          

      opportunity to review/revise the information.                

Scheduling Follow-up Appointments

      Follow-up visits are typically 30-45 minutes. See the Appointment Coordinator at the front desk to schedule 1 – 2       weeks of follow-up appointments.
